of Fallot and abnormal coronary artery: use of aprosthetic conduit is outdated<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b54df7e elementor-widget elementor-widget-text-editor\" data-id=\"b54df7e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t Tetralogy of Fallot (ToF) represents 5\u20137% of all congenital heart\n diseases and is the most common form of cyanotic congenital\n heart disease. Coronary artery (CA) anomalies occur in 5\u201312% of\n ToF cases [1\u20137]. Because this anomalous CA (ACA) most frequent\nly crosses the infundibular region, it can lead to technical difficul\nties during the surgical repair and to increased morbidity and\n mortality rates. A CA lesion can occur if this abnormal\n course was undiagnosed before or during surgery, leading to se\nvere myocardial ischaemia and inadequate or imperfect surgical\n correction of the right ventricular outflow tract (RVOT)\n obstruction [8].\n Several surgical repair procedures have been described in the\n literature to relieve RVOT obstruction without damaging the\n ACA, the most common being insertion of a right ventricle\n (RV)-to-pulmonary artery (PA) prosthetic conduit over the ACA\n or infundibular resection through a right atriotomy and, when\n surgically possible, an infundibular incision either above or below\n the ACA, or both [1, 9\u201317]. An RV-to-PA conduit insertion implies\n future reinterventions, leading to significant surgical cumulative\n risk because the conduit is likely to require revision.\n The twin aims of this retrospective study were to report the\n outcomes for ToF repair with associated ACA in our institution\n and to evaluate the results of different surgical techniques aimed\n at avoiding the use of RV-to-PA prosthetic conduits.\n MATERIALS AND METHODS\n A retrospective monocentric study was conducted in our institu\ntion from 2000 to 2016. We reviewed our paediatric cardiac sur\ngery database to identify the patients with ToF who underwent\n biventricular repair. The Paris V University ethics committee\n granted approval for review of health records. The need for indi\nvidual consent was waived due to the retrospective nature of the\n study.\n From 2000 to 2016, 1132 patients with ToF underwent surgery\n in our institution. We excluded patients with ToF with pulmonary\n atresia and\/or atrioventricular (AV) septal defect, patients who\n had prior surgical repair in another institution and patients who\n could not benefit from biventricular repair. Patients with ToF dis\nplaying a minor infundibular coronary branch were not included.\n On the 943 patients with ToF included in our cohort, 76 (8%)\n had an ACA. An ACA was either diagnosed from preoperative\n transthoracic echocardiography or discovered at surgery.\n CA anatomy is described in Table 1. The main ACA were a left\n CA arising from the right CA (n=47, 61.8%), a major infundibular\n CA (n=15, 19.7%), a right CA arising from the left CA (n=5,\n 6.5%), a single CA from 1 sinus (n=5, 6.5%) or a right CA arising\n from the left sinus (n=4, 5.2%). Elle a \u00e9valu\u00e9 \nl\u2019efficacit\u00e9 et la s\u00e9curit\u00e9 du clip tri\ncuspide chez des patients ayant une \nfuite tricuspide s\u00e9v\u00e8re symptoma\ntique et ayant un risque \u00e9lev\u00e9 ou \ninterm\u00e9diaire de d\u00e9c\u00e8s en cas de chi\nrurgie classique tricuspide. \nLes patients devaient \u00eatre sous trai\ntement optimal et devaient \u00eatre \u00e9li\ngibles \u00e0 un clip (forme faisable en \ninterventionnel). Les patients ont \n\u00e9t\u00e9 randomis\u00e9s entre un clip tricus\npide et un traitement m\u00e9dical (175 \nFinkelstein-Schoenfeld Analysis\n Win ration 1,48\n (1,06, 2,13), p = 0,02\n 12000\n 11348\n patients dans chaque groupe). Les \npatients ont \u00e9t\u00e9 inclus dans \n80 centres aux \u00c9tats-Unis, Canada \net en Europe. \nLe crit\u00e8re primaire \u00e9valu\u00e9 \u00e0 un an \ncomprenait la mortalit\u00e9, une chirur\ngie valvulaire tricuspide, les hospi\ntalisations pour insuffisance car\ndiaque et la qualit\u00e9 de vie. \nLes patients avaient un \u00e2ge moyen \nde 78 ans, et pr\u00e8s de 55 % \u00e9taient en \nclasse III ou IV de la NYHA, et 90 % \navaient une cause fonctionnelle. \nAu total, 87 % des patients ont eu \nune r\u00e9duction de la fuite tricuspide \nau stade mod\u00e9r\u00e9 ou moindre dans \nun suivi \u00e0 un an, versus 4,8 % dans \nle groupe contr\u00f4le. \nDans le groupe clip tricuspide, le cri\nt\u00e8re primaire \u00e9tait obtenu plus sou\nvent avec un ratio de 1,48 (1,06 \u00e0 \n2,13 ; p = 0,02).
